The Geriatrician supports all function of Veteran Medical care in the Community Living Center (CLC) - Nursing home, chronic level of care. This provider also provides support to the Palliative Care and Inpatient Hospice program. In this role the provider is integral to the day-to-day operation of the CLC. Duties and Responsibilities include but are not limited to: Admission of patients to the Community Living Center (CLC) Providing short-term care, long-term care, dementia care, respite care, hospice and palliative care evaluation, diagnosis and consultation Ordering and interpreting tests, prescribing medications, and provision of non-surgical treatment to inpatients or outpatients requiring care to treat illnesses and disorders common to the elderly Managing the appropriate interdisciplinary coordination of the actions of multiple health professionals including physicians, nurses, social workers, dieticians, and rehabilitation specialists in the assessment and implementation of treatment Providing consultative services to inpatient units (medical/surgical floor, ICU/Telemetry, TCU and Rehabilitation inpatient unit Conducting evaluation, consultation, and provision of hospice and palliative care for patients with life-threatening or severe, advanced illness. This includes assessment of suffering and quality of life, managing terminal symptoms, patient/family education, and bereavement Provides patient care through various modalities to include in person, telephone and VA approved telemedicine. Work Schedule: Typical tour of duty is Monday - Friday, 8:00am-4:30pm; however, the position shares calls on a rotating basis with other providers for weekend/holiday coverage. This coverage can be expected expected once a month.
