This HouseCalls NP role will require 80% travel and overnight trips with hotel stay. The NP must travel by vehicle. Counties include: Sullivan, Smith Ulster, Duchess, Putnam, the Bronx, Rockland and Long Island all of Nassau and Suffolk. *map will be shared upon request. (Travel is overnight around 80% 10 weeks per quarter and 2 weeks in home county)

Required Qualifications: Preferred Qualifications: New York Residents Only: The salary range for New York residents is $85,000 to $167,300 per year. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.
